All patients are allocated a named GP when registering with the practice. The named GP takes overall responsibility for a patient’s healthcare, though they may not be the GP who sees the patient on a regular basis. Patients are free to choose to see a GP or other healthcare professional of their choice, providing they are available.
To register please print off the two attached forms or collect forms in reception at either surgery. Once completed the forms need to be returned to the surgery together with two forms of identity (i.e. driving licence/passport) and proof of address (i.e. recent utility bill, tenancy agreement or similar). You need to provide as much information as possible on the registration form to help us ensure you get the best possible care from the start.

There is a new Central NHS Computer System called the Summary Care Record (SCR). Further information can be seen under the link "Summary Care Records" on the left hand side.
For new patients: Summary Care Record patient letter.
You can choose to opt out of the scheme. To do so you will need to complete a form and bring it along to the surgery.
